2. Complete and sign your Living Will form.
- Fill in your decisions for each of the Advance Directive questions.
- Sign your form in the presence of two witnesses. You may also sign with a notary public.
- Be sure to follow the instructions for signing and witnessing the signature included on the form.
3. Give copies to your Healthcare Agent and family members.
- Give copies of the signed form to your Healthcare Agents, family members, and healthcare providers.
- Put your original Living Will document in your End-of-Life Binder.
